The sanctuary dates back to 1200 and is located in Sant'Angelo in Grotte, a hamlet of Santa Maria del Molise. On the perimeter of the old church, a single-nave church was rebuilt from scratch. The walls of the cave were restored and enlarged in 1890 with the contribution of emigrants to the USA. Inside, there is a charming cave with a spring of blessed water. There is a legend attached to this place: "Saint Michael chose this place as his earthly home. Fascinated by the beauty of the place, but disappointed by the behaviour of the inhabitants, one day he walked through a crevice in the rock and from there took flight for the place that is still called Monte Sant'Angelo".

Descending the steps next to the bell tower gives a wonderful view of Monte Miletto.
On the second Sunday in May, San Michele Arcangelo is celebrated. In addition to the sacred rites, there are also popular events and a fair.
